Who knows what it means, "balls to the wall"?
It is the aviation equivalent of "pedal to the metal" in cars. Full speed ahead, or maximum thrust, requires pushing the throttle controls all the way forward. Those have, or at least they used to have, ball handles at the end, so full throttle means "balls to the wall". Nothing to do with human anatomy.
My uncle is retired from the air force. He use to fly the F-111 and did the air show in Topeka wirh the Blue Angels. He told us all about "balls to the walls"

Having a lifetime of uneasy relationship with my body type, I found this story of Cass Elliott (Mamas & Papas, then a brief solo career) to be empathetic and insightful.

He talks about how in any interview, any news rag story, Mama Cass' body was mentioned far more than her extraordinary voice ever was.

Just a few days before her death, Cass performed solo at the 2,300 seat London Palladium, to standing ovations, two sold out performances on two dates. Finally appreciated for her talent, it was her time to leave the world at age 32.

She didn't die of choking on a ham sandwich. There was no sandwich in the room at Harry Nilsson's Mayfair flat where she was staying as his guest (Nilsson wasn't there) when a cleaning lady found her. Some asshole reporter started the ham sandwich schtick, and it was picked up and repeated over and over again before the coroner's report was even released. She died of heart failure after a lifetime of underappreciation by her band mates, record producers and other industry players who profited from her nonetheless, who would rather be preoccupied with her size than with her dreamy singing ability.
