The latter -- everytime a human drinks alcohol, during its processing in the liver, it turns into acetaldehyde before it becomes acetic acid and gets excreted to the kidneys -- the acetaldehyde phase is what makes one feel so ill, during a hangover -- it grows more pronounced and prolonged over time, as the liver becomes less proficient at processing the alcohol and detoxifying the blood; "antabuse" (disulfuram) prolongs the acetaldehyde phase, which is why drinking while on antabuse is both uncomfortable and dangerous.
