I think most of us have encountered it at least a few times.
Most generally (especially on a tank of that design) it is caused by over tightening it and crushing the O-ring.
I recommend purchasing a pair of Vaper Tweezer V4 Multifunction Tools, yes 2 of them.
One for top, one for bottom.
The reason I specify version 4 is the insert.
