Metformin Leads to B12 Deficiency

Dr. David Brownstein, M.D., writes:


Nearly 40% of Americans are obese and more than 60% are overweight, and the obesity epidemic has been growing for decades, so it’s no surprise that many Americans are diabetic.

Metformin (glucophage) is used by patients with Type 2 diabetes to reduce glucose absorption from the intestines, lower liver glucose production, and improve insulin sensitivity. In 2020, there were more than 92 million prescriptions written.

Long-term use of metformin can lead to vitamin B12 deficiency, a condition that is now very common. I have personally seen a steady decline in vitamin B12 levels in patients who took metformin for years.

Thyroid Peroxidase Antibodies and Thyroglobulin Antibodies

Most standard physicals look for downstream effects of autoimmune issues, meaning that levels don’t get flagged until a chronic disorder has already developed. We use TPO and TgAb to detect immune dysfunction at the earliest point and start treatment before symptoms escalate.​
Thyroid Peroxidase Antibodies (TPO) measure immune system activity directed against your own thyroid. When the immune system mistakenly targets thyroid peroxidase (an enzyme central to thyroid hormone production), hormone output gets disrupted and inflammation builds in the gland. High TPO levels are most commonly associated with Hashimoto’s thyroiditis.

Thyroglobulin Antibodies (TgAb) measure a separate but related line of immune attack, this time against thyroglobulin (a protein used by the thyroid to produce and store hormones).​
Why not test one or the other? Elevated TgAb can indicate active autoimmune disruption even when TPO levels seem normal — for example, some people with Hashimoto’s are TPO-negative and TgAb positive. Was talking quite a bit last week about the ever-present issue, which we shouldn’t obsess over, yet need to be hyper-vigilant on at the same time…
