Testosterone is a cholesterol-derived hormone in the body responsible for male sexual differentiation. It is tightly bound to sex hormone-binding globulin and loosely bound to albumin, then about 2% is free and bioavailable.



As a man ages, there's a steady decline in testosterone production or bioavailability, as one of three pathways can happen.

  • Testosterone is converted to estrogens by the enzyme aromatase.
  • Testosterone binds the androgen receptor (AR) on the cell nucleus to produce proteins for cellular function.
  • Testosterone undergoes conversion to 5α-dihydrotestosterone (DHT), a more potent androgen, by the enzyme 5α-reductase. (1)


For nearly seven decades, it's been thought that testosterone, or its metabolites, "is the gasoline that lights the prostate cancer fire."



Briefly, prostate cancer is the fifth leading cause of death from cancer in men, with an estimated 307,000 deaths representing 6.6% of total male cancer mortality. An estimated 15% of the cancers are diagnosed in men, with almost 800,000 occurring in more developed regions. The likelihood of developing prostate cancer increases with age, with the average age at diagnosis being 66 years. (2)



The notion that testosterone participates in prostate cancer development is counterintuitive, as most men have a decline in testosterone as they age. That would mean that the highest rate of prostate cancer is 25 years old, not 66. (3)



So, why do many still think higher testosterone increases prostate cancer risk? Let's begin with a little history.



A Short History of the Testosterone and Prostate Cancer Connection



It all started with the work of Dr. Charles B. Huggins, a urologist at the University of Chicago, around 1940.



Dr. Huggins began experimenting with the effects of castration on dogs with enlarged prostates (BPH). (After all, dogs are the only non-human animals we know of that naturally develop prostate problems. Huggins observed that the dogs' prostates shrunk after castration. But that was not all he observed. Dr. Huggins also noticed that dogs with cancerous-appearing areas also demonstrated prostate shrinkage. When their prostates were removed, the cancerous areas cleared up.



As an experiment, Huggins and his research team then removed the testicles or applied estrogen to a group of men who had metastatic prostate cancer to their bones. At the time, the PSA test did not exist, and a blood test called acid phosphatase, typically high in men with prostate cancer that had spread to their bones, was used to determine the progression of the disease. Indeed, Huggins and his team showed that acid phosphatase dropped substantially within days of lowering testosterone – and injecting testosterone into men with prostate cancer caused acid phosphatase to rise. They concluded that reducing testosterone levels caused prostate cancer to shrink, and raising testosterone levels caused it to grow. (4)





Is Testosterone the Fuel for Prostate Cancer?



In test tubes, testosterone demonstrates an increase in prostate cancer in numerous cancer cell lines, but apoptosis (programmed cancer cell death) once androgens are removed. (5) Again, this suggests that testosterone ignites prostate cancer development in test tubes.



A similar response is found in rat studies: androgens promote tumor progression until androgens are withdrawn – then causing regression of prostate tumor cells. (6) One can easily think from test tubes and rat studies, "That's it, case closed. Testosterone fuels prostate cancer: thus, low testosterone in men is best for prostate cancer prevention."





Human Studies Suggest a Paradox



A meta-analysis of three prospective studies controlling for testosterone, estradiol, Sex Hormone Binding Globulin (SHBG), age, and body mass index (BMI) demonstrated an increase in prostate cancer for men in the highest levels of serum testosterone but no association with DHT or estradiol. (7) Another meta-analysis called the Endogenous Hormones and Prostate Cancer Collaborative Group included 3886 men diagnosed with prostate cancer and 6438 controls. The results demonstrated no direct association between endogenous serum androgens and the development of prostate cancer. (8)



Yet another well-designed human clinical trial looked at 3255 men in the placebo arm of the Reduction by Dutasteride of Prostate Cancer Events trial, also known as the REDUCE trial. Prostate biopsies performed at two and four years revealed no relationship between testosterone or dihydrotestosterone (DHT) levels and prostate cancer risk. (9)



Here's the kicker: There is no causal relationship between high endogenous testosterone and prostate cancer, and low testosterone may cause the disease.



One such clinical trial demonstrated a high incidence rate of an aggressive, more deadly type of prostate cancer among men with low testosterone defined as >7.6nmo/l (220ng/d). (10) Similarly, a group of Chinese men, 110 total, showed greater high-grade prostate cancer (higher Gleason score) in men with low testosterone. (11)



Beyond analyzing staging with Gleason grade on biopsy, a high-risk disease has been associated with low testosterone after prostatectomy. For example, close to 700 men undergoing prostatectomy had their morning testosterone levels taken with surgical pathology outcomes and observed a significant risk of advanced disease that included seminal vesicle invasion in severely hypogonadal men. (12)





Where are we now with Testosterone and Prostate Cancer?



Based on research, it is apparent that optimal levels of testosterone do not induce prostate cancer but are protective against it. Moreover, higher estrogen levels in aging men might be a contributing factor, as shown in some studies (13). This hypothesis is inconclusive as other studies suggest no association between estrogen and prostate cancer. (14) Thus, the role of estrogen in prostate cancer development and progression is complex and multifactorial, incorporating more than one of the mechanisms already described and the interplay between them.





Some patients can benefit from testosterone therapy after prostate cancer diagnosis.



One meta-analysis looking at about 2300 men with low-risk prostate cancer (≤Gleason 6) underwent testosterone treatment and showed short-term safety with no prostate cancer progression regardless of route of administration. (15)





Final Thoughts



It's important to note that prostate cancer is a multifactorial disease influenced by various genetic, environmental, lifestyle, and hormonal factors. At best, there is a weak link between testosterone and prostate cancer, but that is primarily based on test tube and animal studies. Well-performed human studies suggest that if there is any link between the two, low testosterone might increase the risk of prostate cancer, not the reverse. It would be a good idea to have an honest and open discussion about testosterone and prostate cancer with your physician at your next medical visit and learn how you can optimize your testosterone levels after prostate cancer. A conversation on TT in men on Androgen Deprivation Therapy (ADT) is mute since the medical approach intends to lower testosterone to minimize androgen receptor (AR) production. There again, testosterone is not the villain, but the AR's might be. (16)

‘Exercise Snacks’ Build Better Health

If you’re habitually sedentary, your risk of heart attack or stroke is doubled. And the sad fact is only 24.2% of Americans meet the government’s physical fitness guidelines.

Since 1950, sedentary jobs have increased 83%. Physically active jobs now make up less than 20% of the U.S. workforce, down from roughly half of jobs in 1960.

If you’re one of those people whose jobs involve a long day sitting, it’s important that you break up the day with short periods of activity called “exercise snacks.”

Not only will this build up endurance, but your body also creates a collateral blood supply that could save your life if you have a heart attack.

Previous studies have shown that even brief bouts of vigorous exercise, called high-intensity interval training (HIIT), are effective when performed as a single session, with a few minutes of recovery time between the intense bursts. Such training may require a total time commitment of just 10 minutes.

Researchers at McMaster University in Ontario recently set out to determine whether exercise snacks, such as vigorous bouts of stair climbing, could improve a person’s cardiorespiratory fitness — an important healthy marker linked to longevity and cardiovascular disease risk.

They found that the study group who performed such bursts of exercise regularly not only improved their endurance, but their strength as well.

Be aware that if you’re between the ages of 45 and 65, you need to be careful about sudden exertion and always consult your doctor before getting involved in a vigorous exercise program.

“holistic health is hard to afford”
I probably should have made this list on a week where we were talking about a somewhat expensive healing device, Jim

instead of green tea, which is something you might (and should) buy anyway, I got an email similar to those I get somewhat regularly…

about how all "the holistic health stuff you hear about is hard to afford," and why do I even stay on newsletters always talking about something I’m not going to be able to make use of anyway?

Boy, do I get it.

When we were going through everything with Ryder, I had a fairly entry-level job out of college, Teddy had already left hers, and we had debt…

and while we fundraised like crazy (it was my other part-time job, more accurately), and did have family supporting us as best they could, you can only ask for so much without feeling like a total burden…

and all the help in the world doesn’t equate to the totality of what you hear about that COULD be beneficial.

Anyway, with that said, that email reminded me that it’s been a while since I reminded YOU that "the foundational stuff is free."

We were doing every no-cost / low-cost item we could AND buying all the supplements and devices we could, based on how much we could afford at any given time, and how many credit cards we were able to get approved for. ;)

But the bottom line is, there’s a LOT you can do that costs little to no money that all adds up to a MASSIVE impact, so much so that it could very well do the job in and of itself for a whole lot of cases.

Warning: the following list is a collaboration between me and ChatGPT, so some of it might not sound like my voice at times, but every item is coming from me. :)

Do something every day for your mind-body connection—prayer, meditation, breathwork, tai chi, yoga, guided visualization—whatever calls to you. The point is to do something consistently to lower stress, discharge negative energy, and bring in positive energy.

Get outside in nature as much as you can, ideally with bare feet on the ground. Sunshine, fresh air, and earth beneath your feet—these are some of the most ancient medicines we have.

Sprout your own food. Start with broccoli and a few other seeds. Watch our Ultimate Sprouting Kit Guide on YouTube. Eat them by the handful or on everything, and juice the rest. You can grow multiple jars of nutrient-dense food for mere pennies.

Move your body daily. Mix in muscle-building (resistance bands are low-impact, high-reward) and cardio. It doesn’t have to be fancy—just consistent. Your mitochondria and lymph system depend on it.

Eat massive amounts of vegetables. Steam or pressure cook for maximum digestibility with minimum nutrient loss. Pile on the culinary herbs (rosemary, thyme, turmeric, etc.) for flavor and added medicine.

Avoid toxicants like your life depends on it. Because it does. Choose organic when possible, filter your water, stop using synthetic cleaning products, and skip the conventional air fresheners and personal care products—swap in truly natural versions and simplify where you can.

Surround yourself with life-giving, high-vibration elements. Plants, crystals, nature imagery, music that lifts your mood, people that inspire you, objects that bring you peace—everything in your environment matters. Make your space one that supports healing on every level.

Breathe intentionally. Just a few minutes of deep nasal breathing a few times a day, especially slow exhales, can shift your nervous system into healing mode.

Support your circadian rhythm with light hygiene. Get natural sunlight on your skin and into your eyes every morning (no sunglasses, no windows). Avoid bright overhead lights and screens after sunset. Use incandescent bulbs during the day and red lights after the sun goes down.

Sleep like it’s your job. Cool, dark room. No screens before bed. Same bedtime every night. Sleep is when your body heals—don’t cut it short.

Cold and heat exposure (DIY-style). Finish your shower cold or soak in a hot bath. No fancy equipment needed. Both activate powerful repair and detox responses.

Laugh, love, and connect. Relationships, laughter, affection, and purpose—these affect biology as much as food. Hug often. Say the thing you need to say. Watch something funny. Joy is therapy.

Hydrate intentionally. Drink plenty of clean water (add minerals if you have them), and drink it away from meals. Dehydration slows detox and weakens cellular signaling.

Eliminate all self-induced exposure to EMF. Hardwiring your internet connections to your router using a few Ethernet cables can be done in a few hours. Short of that, only turn your Wi-Fi on when you’re actually using the internet (don’t just turn it off at night). Put your cell phone on airplane mode anytime you’re not using it.

Give your digestion a break. Don’t eat constantly. Stop a few hours before bed. Try a 12-hour overnight fast—free and effective for cellular repair.

Feed your soul and mind. Read. Journal. Watch something inspiring. Learn something new every day. A growing mind builds a resilient body.

Next time you’re thinking, "How can I afford all of this stuff?"…

Consult this list and make sure you’re doing ALL of it, because if you’re not, you’re leaving something you could be doing for no money at all on the table…

And again, some of the people I talk to who have healed from pretty serious cases, even when I really grill them on everything they did, I can’t get much more out of them than this foundational type of stuff…

It really can make that big of a difference.
